In 1852, Crumpler moved to Boston and began attending medical school at the New England Female Medical College. She was the only African American student in the school and faced discrimination from both her classmates and professors. However, she persisted and graduated in 1864, becoming the first African American woman to earn a medical degree in the United States.

After graduating from medical school, Crumpler opened a medical practice in Boston. She primarily served the city's African American community, who had limited access to medical care.
